We believe that every child deserves the opportunity to play sports! If you are experiencing financial difficulty and would like to apply for financial assistance to help cover the cost of a sports or fitness program, please download and complete the application below. The opportunity to participate does not have to be held at the ELITE Sports Center.

This application must be completed in full.  If you do not have ALL requirements attached, your application will not be reviewed.  No courtesy calls will be made for incomplete applications.
